COVID-19 Vaccine Information and Registration

The Covid-19 Vaccine is currently being given to individuals in Group 1A who want to receive it. The pace at which we are able to provide vaccinations is dependent upon vaccine availability and evolving State and Federal guidance. It is not mandated that anyone receive the vaccine. All phases are based upon vaccine availability. 

Group 1A: Highest Risk Health Care Workers and Individuals

  • People who have direct contact with COVID-19 patients for 15 minutes or more over a 24-hour period.
  • Long-term care facility staff and residents.

Group 1A COVID Vaccine Request Form

The Covid-19 Vaccine is currently being given to individuals in Group 1B.1. The pace at which we are able to provide vaccinations is dependent upon vaccine availability and evolving State and Federal guidance. It is not mandated that anyone receive the vaccine. All phases are based upon vaccine availability. 

Group 1B.1: Coloradans age 70+, moderate-risk health care workers and first responders.

  • People age 70 and older.
  • Moderate Risk Health care workers with less direct contact with COVID-19 patients
  • First Responders (EMS, Firefighters, Law Enforcement)
  • Correctional Officers (Jail or Prison Front Line Worker)
  • Coroner and Funeral Workers

Group 1B.1 COVID Vaccine Request Form

The Covid-19 Vaccine is NOT currently being given to individuals in Group 1B.2. The pace at which we are able to provide vaccinations is dependent upon vaccine availability and evolving State and Federal guidance. It is not mandated that anyone receive the vaccine. All phases are based upon vaccine availability. 

Group 1B.2: Coloradans ages 65-69, preK-12 educators and childcare workers in licensed childcare programs, continuity of state government.

  • People age 65-69
  • Frontline Essential Worker in Education (Teachers, Faculty and Support Staff of School)
  • Child Care Worker in a licensed childcare center
  • Continuity of State Government (Executive, Judicial, Legislative)

Group 1B.2 COVID Vaccine Request Form

The Covid-19 Vaccine is NOT currently being given to individuals in Group 1B.3. The pace at which we are able to provide vaccinations is dependent upon vaccine availability and evolving State and Federal guidance. It is not mandated that anyone receive the vaccine. All phases are based upon vaccine availability. 

Group 1B.3:

  • People age 60 and older
  • Frontline essential worker in food/agriculture (Grocery Front Line Worker, meatpacking workers, agricultural processing workers)
  • People age 16-64 with 2 or more high risk conditions (obesity, diabetes, chronic lung disease, significant heart disease, chronic kidney disease, cancer, or are immunocompromised, Down Syndrome, heart conditions, pregnancy, sickle cell disease, and solid organ transplant).

Group 1B.3 COVID Vaccine Request Form

The Covid-19 Vaccine is NOT currently being given to individuals in Group 1B.4. The pace at which we are able to provide vaccinations is dependent upon vaccine availability and evolving State and Federal guidance. It is not mandated that anyone receive the vaccine. All phases are based upon vaccine availability. 

Group 1B.4:

  • People age 50 and older.
  • Student-facing higher education faculty or staff
  • Frontline essential workers in Food/Restaurant Services
  • Frontline essential workers in manufacturing
  • Frontline essential workers in US Postal Services
  • Frontline essential workers in Specialized Transportation
  • Frontline essential workers in Public Health and Human Services
  • Faith Leaders
  • Frontline Direct care providers for Coloradans experiencing homelessness
  • Frontline essential journalists
  • Continuity of local government operations
  • Continuity of state government operations
  • Adults who received a placebo during a COVID-19 vaccine clinical trial.
  • People age 16-49 with 1 high risk condition (obesity, diabetes, chronic lung disease, significant heart disease, chronic kidney disease, cancer, or are immunocompromised, Down Syndrome, heart conditions, pregnancy, sickle cell disease, and solid organ transplant).

Group 1B.4 COVID Vaccine Request Form

The Covid-19 Vaccine is NOT currently being given to individuals in Group 2. The pace at which we are able to provide vaccinations is dependent upon vaccine availability and evolving State and Federal guidance. It is not mandated that anyone receive the vaccine. All phases are based upon vaccine availability. 

Group 2: General Public

  • Anyone age 16+
