Care Coordinator (full-time) 7:30am-4:00pm Responsible for care coordination services and functions throughout the organization ensuring that improved quality of care, access to services and resources, as well as improved systems of care is provided for patients. Must foster a culture of patient and family centered care, service, quality, and safety excellence while improving patient and employee experience and community relations. Licensure/Certification/Registration: None Equipment Knowledge: Excellent Computer Skills, Copy Machine and Fax Machine, Printers, Electronic Health Records.

Education: High School Diploma or equivalent GED

Experience: Clerical and care coordinator experience preferred, but not required. Knowledge/Skills/Abilities: Statistical analysis required, advanced verbal and writing skills; knowledge working with databases. Must have verbal skills to present information and ideas essential to supervisory duties and to understand general clinic and practice; capable of dealing with people and working closely with patients; workable knowledge of safety procedures; must be able to read, write, speak and understand the English language; must have patience and enthusiasm as well as the willingness and ability to handle difficult situations with good judgment; must be able to work as a team member; must be willing to learn.
